We go the extra mile in making your first visit to our office convenient, comfortable and informative. The new patient examination consists of a complete oral examination by the doctor, including x-rays and any other records necessary to evaluate the status of your oral health.

A detailed treatment plan will precede any treatment. We always inform before we perform.

If you already have an appointment scheduled and would like to fill out the new patient forms ahead of time, you can download them by clicking on the link below. There should be 5 pages total to bring with you at the time of your appointment. If any of these forms need further explanation, please feel free to leave them blank until you come into our office. We would be happy to help you with them at that time.
